I installed rocky linux on my raspberry pi 4b.
I look at the boot section, there is config-kernel.inc.
I am so curious what the file do.
Now I am installing custom kernel on raspberry pi.
So I want to know that I should the change of contents in config-kernel.inc

From when I used Ubuntu on my RPI4, I never edited this file when installing kernels. I would believe if installing packages/kernels and it required changes to this file it would do it itself.

Yes. it is true.
When I installed Ubuntu on my raspberry pi, there was no config-kernel.inc file in boot.
There is this config-kernel.inc only with rocky linux.

I would just install the kernel, if it needed changes to be applied here, then the package would/should do it.
